Complete Medical History

Recent Laboratory Tests and Results
- Blood test results.

- Ultrasound results.
- Results of specific disease tests (such as HIV, Hepatitis, etc.).
Medical Reports from Treating Physicians
- Specialized reports from the current treating physician(s).
- Physician’s recommendations and opinions regarding the required treatment.
Prescriptions and Medications
- A list of currently used medications and medication history.
- Prescriptions issued by the doctor to be considered in the treatment plan.
Surgical Reports and Previous Operations
- Details of any previous surgeries, including dates and types of surgeries.
- Physician’s recommendations for any future surgeries (if needed).
Pre-Treatment Preparation Guidelines
- Instructions from the doctor on how to prepare for the treatment (e.g., pre-operative or special treatment guidelines).
Vaccination and Preventative Care Records
- A list of vaccinations and preventative treatments the patient has received.
